Tracy Horn Executive Director
Tracy is an entrepreneur and published author with proven expertise in senior transitions, property management, real estate consulting, and not-for-profit initiatives. As a Certified SBA/SBDC small business development specialist, she has built a reputation for excellence in her field.
In 2019, Tracy founded Agyrtis Lending, a private lending firm specializing in hard and soft asset-based solutions. With decades of experience in resort and second home real estate transactions, property management, and staging, she has been guiding families through their life transitions for over 35 years.
In 2021, Tracy leveraged her consulting expertise, outstanding organizational skills, and nurturing personality, along with her Certified Senior Transition Specialists (STS) designation, to assist seniors and developmentally disabled individuals. She helps clients manage the details and logistics involved when downsizing and relocating to retirement living. Tracy earned her MRE and several other designations, which provide a solid foundation for her current endeavors. By coupling her skills, she is expanding her portfolio to include support homes for veterans, recovery, and developmentally disabled adults, all in an effort to address homelessness issues.
Kristinia Clay Care Management Director
Kristinia brings a calming spirit that complements her role as a Licensed Practical Nurse (LPN) and her holistic healing approach to our team. Kristina M. Clay is a seasoned and compassionate LPN with over 17 years of extensive experience in behavioral health, drug and alcohol rehabilitation, and infection prevention. With a deep commitment to patient advocacy and holistic care, she offers a well-rounded clinical perspective that integrates both modern medicine and natural healing.
In her role as an Infection Preventionist and Certified Nursing Assistant Supervisor, Kristina ensures safe, sanitary, and supportive care environments. As a Level One Medication Instructor and Insulin Instructor, she is passionate about education, empowering patients and healthcare staff with the knowledge and tools necessary to improve outcomes and enhance quality of life. Complementing her clinical credentials, she is also a Master Herbalist, which allows her to incorporate herbal and alternative therapies into wellness plans when appropriate, providing a truly holistic approach to health and healing.
With a solid foundation in compassionate care, leadership, and continuous learning, Kristina strives to make a meaningful difference in every life she touches.
Suzette Conner Chief Financial Officer
Suzette Conner serves as the CFO, bringing over 35 years of experience in customer care, administration, process development, and improvement, as well as compliance. She holds a bachelor's degree in Computer Science and has a proven track record of delivering exceptional commitment to clients while ensuring that company operations meet industry standards for compliance. Outside of her professional responsibilities, Suzette enjoys spending quality time with her family and friends and actively volunteers to support caregivers of families dealing with chronic and degenerative diseases.
